Start with two things; how much you're carrying and how long you're wearing it. If you're on site for eight-plus hours with heavy tool pouches, go padded and size up slightly for comfort. If your shifts are shorter, non-padded works just as well. For waist sizing, measure over your work clothes
New grain leather is naturally stiff that's a sign of quality, not a problem. It softens with regular use over the first few weeks. If you want to speed the process up, apply a small amount of quality leather conditioner and work it in gently. Avoid soaking it or leaving it folded overnight.
Both distribute tool weight evenly across your hips and lower back, that doesn't change between the two. The difference is purely what's between the belt and your body. A padded belt has a foam layer built into the back for added comfort during extended wear. A non-padded belt is just without the foam layer.
A well-made grain leather work belt, properly maintained should last several years of daily professional use. The key is leather thickness, stitching quality, and stress point reinforcement. Grain leather handles light moisture well, a brief rain shower or morning dew won't damage a quality leather work belt. For prolonged exposure to water, applying a leather conditioner or water-resistant treatment regularly keeps the leather supple and protected.
For carrying heavy-duty tool pouches, a minimum 2 to 2.5 inch wide belt is recommended. Narrower belts concentrate weight on a smaller surface area which increases discomfort over a long shift and puts more strain on stress points.
